Application Checklist for Toddler – Fifth Grade

• Contact the School office for an appointment to meet with the Principal and tour the school.
• Schedule child’s interview.
• Complete the Application form.
• Attach a recent photograph of the applicant.
• Both parents must sign the application form.
• Enclose registration fee of $400.00

Teacher Recommendation for First – Fifth Grade

• Give the Recommendation form and a pre-addressed stamped envelope to your child’s teacher.
• Give the Authorization for Release of Student Records to an official at your child’s present school with a pre-addressed stamped envelope.

Admission Test for Grades Three, Four and Five

• You will be notified of your child’s assessment date as soon as we receive your child’s records.
